Susanne, I have the same problem with Cleo. She would chew plastic everyday if she was able, the only thing that stops her is me making it unavailable, so I answered 'daily'.

I have completed it twice - once for each of my cats.

The only question I found a bit tricky was the last one re how often witness pica behaviour.  One of my boys is obsessed with plastic, eating and chewing it.  But how often will depend on how often he finds a plastic bag to lick and chew!  So I wasn't sure whether to put every day or what.  I avoid leaving plastic carrier bags lying around.
