Sounds very odd, and possibly congenital since they are siblings - but, I know this is a silly question and hopefully one the Indonesian vet has considered, a 'hole' opening near the genitals suggests some sort of weakness/hernia type of problem rather than cancer ...

Just posted on CC and this seems a very strange thing indeed and sadly after losing one cat the other one has it. As they are related it maybe some genetic thing or something passed on from the mother but I have never read about anything like this before.

So if anyone can help please post on CC or if not a member post here and I will put this link on CC.
