Title: CP volunteering form does anyone have one that I can modify, so not 2 reinvent?

So i dont have to reinvent the wheel would anyone be prepared to share a volunteering form that i can modify for our branch?
We get so many volunteering requests but as usual they dont have a clue what we do or what is to be done and a form would be good. The CP main one is now on line so isnt muchuse..

Any help appreciated.
